How To Send Player’s Proof of Birth (ADULT Registration/Insurance Waiver Forms) Via a Document Attachment
It is expected that for all new player requests, the Proof of Birth (POB) or (ADULT Registration/Insurance Waiver Forms) will be attached to the player's profile as a .pdf file.
Note this is not necessary if a player has been issued a card previously (i.e. a date appears in the Passcard Issued column). In fact, the Browse button on the Upload POB function at the bottom of the individual player’s profile will be disabled for players with cards.
Here's how you do it:
1. Using a scanning device, scan the original or good quality copy of your player's proof of birth document as a .pdf file (Birth Certificate, driver's license or passport). Note that each player must be scanned and saved as a separate file.
-Size restrictions: Restrict the document's size to no larger than a standard sheet of paper (8 ½ x 11)
-Resolution: 72 to 150 dpi
-File overall size: Uploaded document should be no larger than 700KB (this will be plenty of space for your POB documentation).
-File types allowed: PDF, JPG, JPEG, GIF
2. Save this file as a PDF and store it on YOUR computer, using the player's name as the unique identifier (i.e. Smith-John.pdf). Please be sure to note which folder you have saved it in, in order to retreive it later.
3. If you do not have the capability to scan documents, you can rely on the services of Kinko's, Office Depot, Office Max and other printing facilities that provide these types of services for a small fee.
4. Once you have saved the POB (ADULT Registration/Insurance Waiver Forms), you may attach it to the individual player's profile.
-Go to Player Management
-Click on the player's team.
-Click on the individual player.
-Browse and attach the appropriate document (the browse function is at the bottom of the profiles of all players who have not been issued a card previously)
-Update the player by clicking at the bottom (UPDATE PLAYER).
5. When you are ready to request player cards, on the Passcards/Release screen, you can see which players have a POB attached (indicates Attached on the right hand side; or note the View POB to the right side of the birth date on the player’s profile). The POB will be sent to us automatically for our review when you request the passcard. If the POB is accepted by us, Attached will change to Validated on the Passcards/Release screen, and the passcard can be issued.
6. Note that in case of an error, etc, you can change the player’s POB up until the time the POB is reviewed and verified by the US Club Soccer office. Thereafter the POB is locked into the player’s profile until you electronically release the player.
